If you are a new patient to me, scheduling for Pelvic Health, please print and complete the above paperwork before your first visit, or arrive at least 30 minutes before your appointment to complete it.

With paperwork completed we will be able to review your history and jump right in.

If the paperwork is not completed before your first appointment, we will complete it together which may delay treatment to the following visit.
